You are the DESIGNEE for your state team. First, provide us with a little information about your state team's planning. Then list your team DELEGATES and their contact information. Delegates should be designated based on their role in your team’s work related to your state’s goals/ focus areas.
State teams may have at least two (2) and a maximum of eight (8) delegates to participate in the CBI. We recommend teams include a delegate from:
- Vocational Rehabilitation, and
- Special Education.
As your state's DESIGNEE, please provide the requested information below. If you are also one of your state team DELEGATES, please re-enter your information as Delegate 1. Then complete the requested information below for each of your team delegates:
- name
- agency/role, and
- email address.
Once you have completed the form, you will receive a summary of the information you submitted. We will contact your delegates to provide them CBI registration information and updates. Thank you!
