About this Event

Students with disabilities involved in mental health systems, juvenile justice, and foster care often face complex and overlapping challenges that can disrupt successful transition to adulthood. This webinar brings together a cross-sector panel of educators, agency partners, and parent training and information centers who are demonstrating effective, collaborative approaches to support these youth.

Panelists will share practical strategies for building coordinated systems of support, strengthening youth and family engagement, and addressing barriers to education, employment, and independent living. Participants will gain insights into how cross-agency partnerships can improve outcomes and create more equitable, person-centered transition pathways for youth with complex needs.

Learning Objectives:
By the end of this webinar, participants will be able to:

  1. Identify key barriers and challenges faced by students with disabilities involved in mental health, juvenile justice, and foster care systems during secondary transition.
  2. Describe effective cross-system collaboration strategies among schools, agencies, and family organizations that improve transition outcomes.
