Quality outcomes for students and youth with disabilities are achieved from a lifespan approach to transition planning. The services, supports, instruction, and activities change as a person grows older and develops a better sense of themselves and their life and career paths.

These resources can be used to support strategic planning at student, school, or agency planning level.

Predictors of post-school success are evidence-based, research-based, and promising services, supports, and/or acquired skills associated with improved post-school outcomes for students with disabilities.
